
Foundayo® is a once-daily oral GLP-1 treatment for weight management manufactured by Eli Lilly.
Unlike injectable GLP-1 treatments, Foundayo is taken as a tablet and provides an additional treatment option for adults who are clinically suitable for prescription weight-management treatment.
The 0.8mg starting strength is currently available, with higher strengths being released progressively by the manufacturer.
Treatment can only be prescribed following an individual clinical assessment.
What is Foundayo?
Foundayo contains orforglipron, an oral GLP-1 receptor agonist.
It is taken once daily as a tablet.
Six strengths are licensed in the UK:
0.8mg · 2.5mg · 5.5mg · 9mg · 14.5mg · 17.2mg
Treatment begins with the 0.8mg starting strength.
Any subsequent dose increase is determined by the prescribing clinician according to the licensed prescribing information, the patient's response and tolerability.
What does the clinical evidence show?
Foundayo's active ingredient, orforglipron, has been evaluated in a programme of Phase 3 clinical trials involving thousands of participants.
Average weight loss of 11.2% at 72 weeks
The ATTAIN-1 Phase 3 trial, published in the New England Journal of Medicine, included 3,127 adults with obesity or overweight and a weight-related health condition who did not have diabetes.
Participants received once-daily orforglipron or placebo alongside healthy diet and increased physical activity.
At 72 weeks, average weight change was:
At the highest studied dose:
- 54.6% of participants lost at least 10% of their body weight
- 36.0% lost at least 15%
- 18.4% lost at least 20%
Improvements were also observed in measures including waist circumference, systolic blood pressure, triglycerides and non-HDL cholesterol.
Individual results vary, and the results of a clinical trial cannot predict how much weight an individual patient will lose.

Could an oral treatment help maintain weight previously lost on injections?
An important area of research is what happens after patients have successfully lost weight using injectable GLP-1 treatments.
The ATTAIN-MAINTAIN Phase 3 study investigated patients who had previously completed treatment with either semaglutide (Wegovy) or tirzepatide and had reached a weight plateau.
Rather than simply stopping GLP-1 treatment, participants were switched to once-daily oral orforglipron or placebo and followed for a further 52 weeks.
After previous Wegovy treatment
Participants had reduced their average weight from 113.5kg to 95.0kg before switching treatment.
After another year taking oral orforglipron their average weight was 95.9kg.
This represents an average regain of only 0.9kg and, based on the reported mean weights, approximately 95% of the previous weight reduction was retained.
After previous tirzepatide treatment
Participants had reduced their average weight from 115.8kg to 90.9kg before switching.
After 52 weeks of oral orforglipron their average weight was 95.9kg.
This represents an average regain of 5.0kg and approximately 80% of the previous weight reduction was retained based on the reported mean weights.
These findings suggest that oral orforglipron could potentially provide another longer-term treatment option for some patients who have previously achieved substantial weight loss with injectable therapy.
Important: ATTAIN-MAINTAIN is a Phase 3 study for which Lilly has reported topline results. At the time of writing, the detailed results have not yet been published in a peer-reviewed medical journal.
A clinician should assess whether switching treatment is appropriate for an individual patient. Patients should not stop or change their existing weight-management medication without discussing this with their prescriber.

Already taking Wegovy or Mounjaro?
If you have successfully lost weight with an injectable treatment but would prefer to explore an oral option, you can discuss this with your clinician.
Research into switching from injectable GLP-1 treatment to oral orforglipron is encouraging, but whether a switch is appropriate depends upon your individual circumstances.
Do not stop, reduce or change your existing medication without first discussing this with your prescribing clinician.

Where prescription treatment is clinically appropriate, you and your clinician will agree a treatment plan.
Follow-up reviews are normally approximately 15 minutes and allow us to assess treatment response, side effects, tolerability and whether treatment should remain unchanged or be adjusted.
